The service will need to receive a referral from the clients GP before a client can be placed on the waiting list. This would include:
A completed referral form from the responsible GP, this is to support a referral into the service and show commitment to shared care arrangements for hormonal treatment. We require GP surgeries to administer injections. The service prescribing team will be responsible for assessing and recommending treatment, and will provide monitoring until the client is stable on hormone treatment, administrating the treatment will need to take place locally.
The client will be discharged from the prescribing clinic back to their GP once a client is stable on hormone treatment or is discharged from the Gender Identity Service.
The GP would need to provide an up to date physical examination report and blood test results. The blood tests required are: FBC, U&E’s, Gamma GT, Serum, Calcium, B12 & Foliate, Cholesterol, Triglycerides, Fasting Blood Sugar, TFT’s, SHBG, FSH, LH, Prolactin, Testosterone and Oestradiol. In addition to the physical examination an examination of the sexual characteristics would be highly desirable.
